Annual Wellness Assessments

Welcome! The City of Raleigh’s Benefits Division in partnership with Provata Health is offering all medical plan participants the Annual Wellness Assessment (AWA). The AWA consists of biometrics screenings, an online health risk assessment (HRA), and a tobacco attestation statement. By participating in the AWA process and completing all the eligible components, you will be considered “wellness compliant” and earn the following premium discounts:

NOTE: Retirees and covered spouses who participate in Medical Plan B are automatically considered “wellness compliant.” However, you are highly encouraged to participate in the AWA process for your own personal awareness.

New This Year:

 

  • If an Employee has spousal coverage, both the employee and the covered spouse must complete the AWA requirements to be eligible for the health insurance premium discount with the plan year beginning January 1, 2018.

  • If a Retiree on Medical Plan A has spousal coverage, both the retiree and the covered spouse must complete the AWA requirements to be eligible for the health insurance premium discount with the plan year beginning January 1, 2018.

  • If a Retiree on Medical Plan B has spousal coverage and plans to switch to Medical Plan A during the Open Enrollment period, both the retiree and the covered spouse must complete the AWA requirements to be eligible for the health insurance premium discount with the plan year beginning January 1, 2018.

The AWA Process:

PHASE I (August 1 - September 1)

TOBACCO ATTESTATION:

Everyone must complete the Tobacco Attestation Statement. You must affirm one of the following:

  • I certify that I am tobacco-free, therefore making me eligible for the tobacco-free premium discount.
  • I certify that I am not tobacco-free, therefore making me ineligible for the tobacco-free premium discount.
  • I currently use tobacco and agree to participate in a tobacco cessation program. I understand that to qualify for the tobacco-free premium discount for the January 1, 2018 to December 31, 2018 benefits plan year, completion of a certified tobacco cessation program is required by November 20, 2017.

TOBACCO-FREE means you have been tobacco free for at least three months and will not use tobacco through December 31, 2018.

TOBACCO is defined as any form of tobacco products that are smoked, applied to the gums, chewed, or ingested, and/or inhaled. Including - but not limited to - cigarettes, cigars, pipes, tobacco leaf derivatives like dip or chew, snuff & electronic cigarettes.

PREPARING FOR YOUR BIOMETRIC SCREENING:

Participants should attend their scheduled times and locations to complete the AWA process. If you are unable to attend your scheduled AWA screening, you must make plans to attend another event with the exception of First Responder Only events. You may contact the Human Resources Department at 919-996-3315, if you have any questions or require additional information.

  • Fast for 9-12 hours ahead of your screening.
  • DO NOT eat or drink anything except water. Black coffee or tea is also fine, but do not use cream or sugar.
  • DO take all medications as directed by your physician(s) as you normally would.
  • DO drink plenty of water before your screening to stay hydrated.
  • If you have a medical condition that does not allow you to fast for 9-12 hours, please follow your healthcare provider’s instructions.
  • DO wear a loose fitting shirt that allows you to roll up your sleeve for a blood pressure check.

PHASE II (Early September):

2017 Annual Wellness Assessment Screening Schedule

The dates for completing all AWA requirements has been extended until Friday, September 1, 2017. In order to give participants additional opportunities to complete all AWA requirements, two make-up dates and extended hours have been added to the schedule for screenings.

*Hours for Friday, August 25, 2017 have been extended until 2 p.m. (walk-ins)

You may now schedule a screening on Thursday, August 31, 2017 and Friday, September 1, 2017, 7 a.m. - 2 p.m.
