Accessible Raleigh Transportation Trip Request Forms

Tier II service is available to Accessible Raleigh Transportation (ART) users who quality under the Americans with Disabilities Act (ADA).

Please fill out the appropriate form listed below. The Online Versions will be submitted to the ART group. The Microsoft Word Versions must be saved to your computer and then e-mailed as an attachment to ART@raleighnc.gov.

*E-mails are accepted for ART Tier II trip requests from 8am to 5pm Monday through Friday except for Holidays.
E-mails sent after 3pm will be sent to an overflow provider.

When using the Microsoft Word Version click on File and then Save As. Open the saved document and fill it out. When you open a blank form your cursor will be in the first empty field (gray box) beside "Taxi Company". Start typing and you will fill out this field. To move to the next field you may hit tab or click in the next empty field. After you have filled the form out click Save and then attach the saved document to an email addressed to ART@raleighnc.gov. To help expedite your trip request please do not hand write on this form.
