Utility Billing Account Request

Making plans to move to a new home? Making plans to move out? Changing banks?

With this form you can start and stop service, enroll and change AutoPay information, or update your account information when things change, such as a mailing address, phone number, email address, authorized person, etc.

The following form has seven sections, but not all sections are required.

  • Start Utility Services on New Account
    Complete Section 1, Section 2, Section 3 (optional), Section 5 (optional) and Section 6.
  • Stop Utility Services on an Existing Account
    Complete Section 1, Section 4 and Section 6.
  • Update Utility Account Information
    Enter information in any fields that need to be updated.
    Complete Section 1 and Section 6.
  • AutoPay (Enroll or Change Recurring Bank Draft Information)
    Complete Section 1, Section 5 and Section 6.

Online forms are provided for our customers’ convenience. You are not required to use the online option. If you prefer to print and mail paper forms to the City of Raleigh, these forms are available in PDF format. Download printable forms.
