Annual Wellness Assessment

The 2018 Annual Wellness Assessment (AWA) period is April 30 - Aug. 3. This is your opportunity to participate and become eligible for a health insurance premium discount and gain valuable information about your health.

There are three components to the AWA process:

  1. Online tobacco attestation;
  2. Online health risk assessment questionnaire; and
  3. Biometric screening

To be AWA-compliant and receive a discount on your medical premiums, employees and their covered spouses (if applicable) are individually required to complete all three AWA components by August 3.

Participation in the 2018 AWA process will apply to the 2019 Medical Plan Open Enrollment.

RETIREES

The Annual Wellness Assessment also applies to City of Raleigh retirees. Retirees and retiree spouses enrolled in Medical Plan A must individually complete all three components of the program to be considered AWA-compliant. Retirees and retiree spouses enrolled in Medical Plan B are not required to participate in the AWA, but are encouraged to do so for their own health awareness.

Let's Get Started - 1. Online Tobacco Attestation and 2. Online Health Risk Assessment Questionnaire

Beginning April 30, you may go to Raleigh Wellness For Life and complete the online tobacco attestation and online health risk assessment questionnaire. You must login using the same credentials as 2017 or signup to create an account.

NOTE: Both Components 1 and 2 must be completed prior to attending a scheduled screening appointment.

Final Component - 3. Biometric Screening

Screening Option:
Schedule your appointment for a biometric health screening. This is a check-in on important health indicators like blood pressure and cholesterol. The City is providing on-site biometric screenings between July 9 and Aug. 3. There is a schedule of biometric screening dates and times below, as well as, a link to a printable download of the schedule.

MANDATORY: You must make an appointment in order to attend one of the City’s on-site biometric screening session. To make your appointment, go to Raleigh Wellness for Life.

Primary Care Provider (PCP) Option:

If you prefer to have your screening done by your Primary Care Provider (PCP), download the PCP form on the Wellness for Life website. Your healthcare provider must complete the entire form. You may submit the completed and signed form to Provata Health using one of the following:

  • In-Person: Drop-off the completed and signed form during an on-site biometric screening event.

How We are Communicating

Here’s How We are Communicating with Employees and Retirees about the 2018 Annual Wellness Assessment Process:

  • Mailing to all employee and retiree home addresses: Look for your information packet from the Human Resources Department with step-by-step instructions on how to be AWA-compliant. Packets were mailed to all active and retired City of Raleigh employees the last week of April 2018.

  • Automated phone calls from the Human Resources Department will go out to all active and retired employees enrolled in the City’s Health Plan with reminders about completing the AWA process by August 3rd to realize savings in monthly health care premiums. ACTION ITEM: In order to receive official City messages, please check to be sure your correct home or cell phone number is listed in PeopleSoft.
